The switch to Tinzaparin is happening next week

MFT will switch from dalteparin to tinzaparin for adult patients on the morning of Tuesday 15 September 2026.

Complete the training

All colleagues who prescribe, administer or supply tinzaparin must complete the e-learning module by Friday 11 September 2026.

When will tinzaparin stock arrive on the wards?

  • Stock has already been delivered to some wards, but for most areas the stock will arrive during the week beginning Monday 7 September 2026.
  • Contact pharmacy if your stock has not been received by Thursday 10 September 2026.
  • Dalteparin will remain available in the Emergency Drug Cupboard on each hospital site.

What should happen to current prescriptions?

Adult inpatients: Prescribers should change dalteparin prescriptions to tinzaparin on the morning of Tuesday 15 September 2026.

Adult outpatients: prescribers should change dalteparin prescriptions to tinzaparin when the patient’s next prescription is due.

Which patients should not be switched on Tuesday 15 September?

  1. Obstetric patients already established on dalteparin treatment doses should remain on dalteparin.
  2. Adult patients already established on treatment doses of dalteparin, adjusted based on anti-Xa monitoring or following ACS, should remain on dalteparin until the course of treatment is complete.

Information for patients

A patient information leaflet is available to help explain the change from dalteparin to tinzaparin.

Patient safety

The introduction of tinzaparin represents a significant change in practice across MFT. To minimise the risk of dosing errors, all prescribers, nurses, midwives and pharmacy staff must check every tinzaparin prescription against the dosing guideline before prescribing, supplying or administering the medicine.

Pharmacy will provide every ward with copies of the tinzaparin dosing guideline by Friday 11 September 2026.

To support the safe and correct prescribing of tinzaparin, a number of new or updated order panels will be available in Hive from the morning of 15 September.

For inpatients, the following order panels will be available by searching for ‘Tinzaparin’ or the panel name below:

  • VTE Prophylaxis for Obstetric Patients
  • VTE Prophylaxis for Adult Patients
  • VTE Treatment for Adults (use for both obstetric and non-obstetric patients)

For discharge and outpatient prescribing, the relevant panel supports prescribing of multiple syringes where clinically appropriate and includes default patient instructions to support correct administration. Find the panel by searching for ‘Tinzaparin’ or the panel name ‘Tinzaparin (Leo) pre-filled graduated syringe order panel (includes initial VTE treatment dosing)’

Personal Preference Lists

If you have previously added dalteparin orders or order panels to your personal preference list, please review and update these and replace them with the appropriate tinzaparin orders/panels where necessary.

Spotted an Issue?

If you notice any references to dalteparin in Hive that you believe should instead refer to tinzaparin after the go-live, please log a service desk ticket so that the issue can be reviewed and corrected.
